Helping you stay healthy, live longer and stay out of hospital
We want everyone in West Sussex to live longer and enjoy healthier lives.
From December 2009, GPs in West Sussex have been looking at NHS healthcare records to see who could benefit from more help to manage their health.
Just as weather forecasters use computer systems to predict the weather more effectively, healthcare staff are using computer systems to predict who might be at risk of becoming ill enough to need emergency admission to hospital.
By gathering and analysing information from healthcare records, we can help you to stay healthier, look after you better and keep you out of hospital – before emergencies arise.
As well as helping us to plan healthcare services that best meet the needs of patients, the information that is being reviewed will also enable us to check that we are delivering the most appropriate care.
This service will be great for patients, and keep more people healthy and out of hospital.
What this will mean for your care
- We will help you to improve your health and wellbeing through easy and timely access to practice- and community-based healthcare services.
- We will offer a more personalised health plan for those patients who would benefit from this.
