Proud to Care Nursing Awards 2012-13

Join the celebration of nursing in Sussex by nominating a nursing / care giving team or an individual nurse, health visitor, midwife or care giver for a Proud to Care Nursing Award.

The Public’s Choice Award

NHS Sussex invites nominations from patients, their families or carers for the Public’s Choice category in the Proud to Care Nursing Awards 2012-13.

If the team or individual looking after you or someone close to you has…
• gone above and beyond the duty of care, demonstrating compassion, competence, excellent communication, courage and commitment to ensuring care is the best it can be, or
• worked with people, carers, families and staff to enhance the experience of care, or
• been exceptional in any way…
nominate them for the Public’s Choice award.

You can do this by:
• writing a Proud to Care postcard (no stamp needed) - cards have been distributed to hospitals, surgeries and other places where nurses and care givers work
• writing to NHS Sussex at Freepost RLZL-AJKR-XGRZX, Level 4 Lanchester House, Trafalgar Place, Brighton BN1 4FU
• emailing ws-pct.proudtocare@nhs.net
• downloading the Public’s Choice Award nomination form below 

Include the name and organisation of the individual or team you are nominating and why you think they deserve the award (up to 100 words).  You can give your name and telephone number / email address but this is optional.

The closing date for nominations is 21 February 2013

 

Nominations will be shortlisted by a panel of judges.  Shortlisted nominees will go forward for judging by Sussex directors of nursing, a patient representative, specialist health professional and a communications lead.  Awards will be presented at the Proud to Care Nursing Awards evening on 25 April 2013.

Staff nominations for the Proud to Care Nursing Awards 2012-13

You can nominate a colleague or a team for up to six Proud to Care Nursing Awards:

Individual award nominations must be for a nurse, health visitor, midwife or care giver in any care setting – acute, community, primary, council or private. 

Teams can be made up solely of the above or any combination of these health professionals and others who form part of a wider multi-disciplinary team.
